Please take the time to read through this information carefully.

Please note that by making an appointment with Wellsense, for any population health consultancy or functional nutrition or medicine or mind body therapy offered, you agree to these Terms & Conditions and Privacy Policy in their entirety. You also agree to attend the appointments/sessions you have booked.

Appointments

Appointments vary in duration depending on treatment type and individual preferences and needs. If this is your first appointment, you will receive a New Client Consultation Form, Medical Symptom Questionnaire and Medical Intake form and Agreement which must be returned no less than 3 days prior to your appointment. Please ensure that you complete this step -it is a necessity for completion of your appointment and treatment. 

Cancellations and late arrivals

Cancellation Policy: 24 hours. Please cancel as soon as possible if a foreseeable event is likely to prevent you from attending, for example, illness or adverse weather conditions. I require a minimum of 48 hours notice to cancel an appointment. Cancellation with less than 48hours notice will incur a charge of 50% percent of the appointment fee. An invoice for any cancellation / ‘no show’ charges will be sent to you by email for payment by bank transfer, due within 7 days. 

Late Arrivals: It is important that whether in person or virtual, our agreed appointment time and missed time cannot be made up by overrunning or adding time to subsequent appointments.

Payments

  • Payment is expected before the appointment

  • I accept cash, mpesa or bank transfer

Confidentiality

All client information and records will be safeguarded by Monique Oliff and will remain confidential. Unless required to do so by law, I will not otherwise share, sell or distribute any of the information you provide to us without your consent.

Health and Safety

Clients are asked not to attend any in-person appointment if they are unwell, suffering from a cold or virus, infection or general ill health. If you are unwell and contact us to inform me of your illness, please note that my Cancellation Policy may still apply. If Monique Oliff cancels due to illness

Monique Oliff holds the right to refuse any treatment if a client is physically or verbally abusive towards her, or demonstrates inappropriate, aggressive or sexual behaviour. Such behaviour may result in criminal proceedings.

Disclaimer

Functional Nutrition and Medicine is not a substitute for professional medical care. Monique Oliff will always recommend referral to a GP or other qualified medical specialist or practitioner or mental health professional where appropriate. You/the client has a duty to provide accurate and true medical and personal information, to the best of your knowledge, and to keep Monique Oliff up to date with details of any medication, symptoms, mental health or medical concerns or treatments you are having investigated or undergoing treatment for.

Functional Nutrition and Medicine is not substitute for professional conventional medical care or counselling. Functional Nutrition and Medicine is complimentary and is not appropriate for acute life threatening illness and disabilities. Functional Medicine and Nutrition is a process in which you play an active role, you will get out of it what you put into it. If you have a serious medical condition, please consult with your GP. Monique Oliff may refuse to treat any client with just and reasonable cause. Monique Oliff reserves the right to terminate any appointment at any time if they believe that it is not safe or in the client’s best interest to continue. Likewise, the client has the right to refuse, modify or terminate treatment at any time, regardless of prior consent given.

Medical Waiver & Disclaimer:

Monique Oliff provides population health and functional nutrition and medicine therapy services. You/the client agrees to provide any known medical or mental conditions that might be affected by the services. It is the responsibility of you/the client to disclose any medical conditions, including but are not limited to: Heart conditions, stroke, diabetes, skin conditions and cancers.

Indemnification:

You/the client agrees to indemnify Monique Oliff and Wellsense Consulting, officers, contractors and employees, harmless from any claim or demand, including reasonable attorneys’ fees and costs, made by any third party due to or arising out of use of any person or facility provided by Monique Oliff T/A Wellsense Consultancy

Inappropriate Behaviour

  • Monique Oliff has the right to terminate a treatment immediately if the client demonstrates inappropriate behaviour.

Cancellation Policy

  • A booking is confirmed once Monique Oliff and the client have both agreed a date, time, duration and location for a treatment

  • If a cancellation is made more than 48 hours before a treatment, the client will not be charged

  • If a cancellation is made within 48 hours of the treatment, client will be expected to pay a cancellation fee of 50% of the appointment cost.

  • This cancellation fee may be waived at Monique Oliff’s discretion.

  • The cancellation fee must be paid within 7 days of the cancellation.

  • If a client is having an in-person appointment and is experiencing respiratory symptoms, the client must cancel their session with immediate effect and the cancellation fee may be waived with confirmation of illness.

  • Monique Oliff has the right to cancel a treatment that has not been paid for in advance by giving 24 hours notice

  • Monique Oliff has the right to cancel a treatment without notice if she is taken ill. 

  • Monique Oliff commits to rearranging treatments to a new time and date within two weeks of the cancelled treatment where possible
